Mazzoni Center is asking for your help, as we survey the health needs of the LGBT community and other populations in the Philadelphia area.  

We've developed a brief, anonymous online survey to learn more about the demographic factors of the populations we serve and those living in our nearby neighborhoods, including: age range, languages spoken, socioeconomic and housing status, health insurance status, ethnicity/culture, health status, and health care utilization patterns. We're also looking at perceptions about healthcare as well as barriers to accessing needed services.  Your responses to these questions will help us provide essential healthcare services to our community.
